Being onsite, the consultant can closely monitor the patient's progress and facilitate solutions to address medical needs. Within guidelines, CCC's case management consultant works to insure the patient receives the highest level of care for the patient's health dollars or other funds being spent for care of the patient by employers or healthcare providers.

Telephonic case management provides high quality case management services, administered by professionals certified in case management. If cases can be managed without the need for onsite visits, further cost savings may be realized by employers or healthcare providers over onsite case management, while maintaining high standards of care coordination. With telephonic case management, service agreements between employers or healthcare providers and CCC will guarantee timely initial response and followups to oversee management of health care needs.

Task assignment case management is a customized program whereby the client, the patient and/or healthcare provider works with CCC to detail out specific requirements for healthcare management. Such programs vary based upon need but represent an overall approach to care managment with specific goals and objectives. As with onsite and telephonic case management, CCC provides certified case management healthcare professionals to administer the programs and insure that quality is maintained.

Where patient, employer, or healthcare provider requirements dictate extraordinary circumstances which don't fall under one of the other general categories of case management such as one-time requirements, special attention needs, et cetera, CCC will be happy to work with patients, employers, and healthcare providers to create a program to address these special needs - whether they be special reporting requirements, specific tasks to be undertaken that don't otherwise fall under task assignment case management, or legal obligation issues.
